It's a chin strap.
Yeah, chin strap for an old football helmet. I remember seeing patients in places like this wearing those to protect their heads.
Maybe they did have helmets as well as the mask. It looks like a chin strap to me too.

I have a strap like that in my Lacrosse helmet
It's a chin strap for a helmet used to prevent head banging. When I worked in a pediatric psych hospital, you'd occasionally have to put violent kids into holds to prevent injury to self or others. If they started banging their head, you'd use a modified sort of foam football helmet. Or your hand. I broke a knuckle like that.
Definitely a chin strap, but I was thinking that maybe it was a restraint for patients who were "Biters"? I dunno...
