Is that on the inside of a door? I don't see a handle on the door. Do you think its patient writing or graffiti.

Either way they have a lot of time on their hands!
I would guess and say patient writing. the words seem pretty rusted, and I feel if someone was going to graffiti the place, they'd use the tools of the trade. But hey, I could be wrong.
Do you seriously think a hospital would allow patients to spend hours carving things into a door and then just leave it there? If it were a small amount of writing on a wall, I could see it being missed by staff and maintenance crews. But this is rather significant grafitti that wouldn't go unnoticed.
They didn't care about their patients I doubt they would care about the walls. Especially if it happens often since the patients were treated worse than animals.
The staff claim the patients would hurt others. Than why care if the patients wrote on the wall since they would kick the walls anyway.

One of the words I make out on the wall says nurses.

There is no way that is graffiti you can barely understand what it says for starters, and the rust indicates it has been there a long time. The paint was probably chipped off by the patients fingernails. I don't see any writing marks from any writing tool.
