Like in Germany - a form for everything..
Life revolves around forms in hospitals. There seems to be a form for anything and everything. Some of the forms I have dealt with were signed by a doctor in pencil! One would think that they would know that they have to sign in pen.
"...So, did you die instantaneously or did it take a few minutes?...Instantaneously, I see....Now, mother's maiden name..."
I like how it has "Stick bar-code here" in the right corner.

"BEEP! So will you need a carry out today?"
I know you are dead, but can you fill this out for me?

Blood?! Paper?! =O This is too scary for me.. *faints* x.x............. xD
Yep, healthcare in general is notrious for paperwork. That is all my job really is, "pencil pushing." But it is more of a liability issue than anything. Also though, that information collected is used in a benefical way. The statistics collected help research determine what areas need attention the most and what the common ratio for certain illness, injuries, mental illness and accidents are. One has to know where to begin helping before help can be given. I think this form was used mostly to document collection samples taken from the corpse at the time of autopsy. Of couse a dead person can't answer a survey.
You'd think It would be kind of hard for a deceased person to take a survey.
