It's a two in one. The hole in the middle is where the soap comes out. Can't drop the soap this way, it pours out on you while you're showering.

Notice how the shower head doesn't hang all the way forward? Well there's a reason for that. The smaller the shower head the harder it is to tie a sheet around it....and thereby thwarting any suicide attempts.

Actually, that's a Speakman showerhead, quite a good showerhead. Stem-mounted Speakman showerheads can be found in Marriott and Westin hotels, as well as the White House.

I've been in four different hospitals and the shower heads all look almost exactly the same as this. Some are even smaller, and yes, they're designed to prevent suicide attempts. The shower floors are mostly of the locker-room variety, though at least now people are given a modicum of privacy.
