This might sound strange (probably will X^D) but did you try sniffing the stain? It could help in identifying it. I doubt it's poop or blood or anything, I can guess it could be paint or something.
I really think it looks like someone's hand was bleeding and they wiped it on the wall...it looks like really old dried blood to me...then again I wouldn't know what really old dried blood looks like... *shifty eyes*

I don't know if anyone could tell what this is without having samples examined in a lab, but I for one don't wanna know what it is!
It does, indeed, resemble dried/smeared blood, but don't hold my word to that. If it were to be that, then methinks it would have been cleaned unless it occured after the shut down of this ward.
I must admit that I did think of the Poopkid when I first viewed this image. Haha. I just can't help myself sometimes.
Perhaps it's a painting which interprets a lighting fixture?
I thought perhaps it was some form of corrosion or rust, but the origin point appears to have been applied, rather than from dripping down the wall...
Blood? I hope so, but that would be too good to be true. I hate to sound like a "crazy" person,but i have the word "blood" written in blood on th back wall of my closet, this reminded me of it, nice color contrast by the way. Great shot as always.

I would like think...blood...because more thrilling! However, could it be that some kind of cabinet sat against the wall and it maybe rusted and left that mark...yet again, the "crazy" me like think...blood...whereas the sensible me think...object. hehe.
I dont think blood and I am nuttier then a fruit cake.. what I think when I look at this picture is someone was trying to entertain themselves and decided to leave a finger paint portrait in poop.

OK whats with the wall, it looks like it was trying to eat the patient. Hence, the blood.
its either blood or the work of a disgruntled fecalpath...or the sign of a VERY hungry wall
im not really sure what it is i remember there was dried cat poop on my basment wall but it crusted and flaked like the whit epaint in the picture and blood jsut stains and it is a mental hospital and ive seen in a documentry a patient who was in a straightjacket in one of these rooms and he kept hitting his face into a wall and his eyebrow was bleading and he wiped it on the wall and it looked almost the insact same...... kinda chilling reminder if you want to take a look at the movie for your self its called Faces Of Death 2
Its either a dirty protest or smeared blood, for sure...

But to be honest, I'd say it was blood. Sometimes I do blood paintings, and the stain does look very similar to the dried blood of my paintings. (I'm not as strange as I sound by the way...)

No one can for sure unless you can smell it - if it even has a smell after all this time. But it does look very similar to when my cat dragged his ass across the rug so I would have to vote for poo - -:)
Did you 3 out of 4 urban spelunkers can't taste the difference between blood and poop? An uncultivated palate is such a shame.
The slight reddish colour tells me this is blood. I bet it was left by the very last occupant of this cell. Since it was never cleaned, it could have happened when they came to transport the patient out, when this ward was being closed.

Or... might be from a vagrant or vandal who was here, more recently.
that looks more like a smear than a splatter - I think we're all getting just a little bit too involved in identifying this - :)
Maybe . . .

As they were transporting the last patient out, the patient got angry or had an episode, causing them to bash their head against the wall. . .

and smeared it into a stain?

It looked pretty much like dried blood...
The same when I put my own blood on the paper and watched it dry. It looked just the same (the color and all)!
Maybe someone wasn't ready to leave when they closed.
