I would like to believe it's a spectre of a former patient who is most grateful to Motts for documenting this place of horror as testimony of the truth.
it looks like Motts with the light behind him casting a shadow. If you look close you can see it on the bar also.

it seems like even the bathrooms would have nice little catwalks across the top so that nurses could "study" the patients even further.
it all sounds like some horror story where the doctors are being held captave in this hospital and constantly being survalenced by "nurses" that are actualy secret agents and all the while the patients are kept blissfully unaware, but if one of them do find out, OFF TO THE BLACK AND BLOO ROOMS WITH YOU!
again, very 1984ish.
