They appear to be fastened BOTH to the wall -and- the ceiling! Real '40s industrial tech design though...
Looks like they didn't want anyone swinging from those fixtures! Gotta love the "backup" light underneath it!

Thats not a soap dispenser look off to the right there is a pipelike wire running horizonally then it goes down vertically.
RBV, the soap dispenser comments are a running joke on opacity.

The lower light was added later. Notice that the wire is run on the outside of the wall.
We find soap dispensers in the oddest places...
The perspective threw me at first, I couldn't figure out why there was a soap dispenser and whatever else that was sticking up out of the floor.

I would agree Daniel. Maybe not a lot later, but it wasn't installed with the first one. I'm thinking they installed it after a dark spot showed up-maybe somebody fell or even got attacked by an inmate.
Maybe they installed it because the florescent light kept flickering. (or maybe I watch too many horror movies) ;-p

Wow, another "Neat old light" soap dispenser! Wonderful perspective.
Maybe the lower one is an emergency light that comes on in the event of a power outage? Or it could be a soap dispenser--you know how many germs there are from all those hands touching the railing.

I think the top large item is not a light but is a heater, similar to those one sees at skating arenas. The smaller item below is a light.
electricians call the lower light a "jelly jar". they make great drinking glasses when your poor.

There is something unsetteling about this photo.
Im not sure i understand the whole tiny light down below.....the big one seems like it would have overpowered the smaller one anyways.....hmph....
My guess in the lower light wwas installed soemtime in the 1970's since I have two just like it on my house which was built in 1978.
