I think the dark room was for the development of x-rays. Many state run hospitals were self-sufficient, so they didn't have to take the patients off site. (ie. ) doctor, dentist, barber hairdresser.
The porcelain "thingi" in the middle is the drain stopper. The idea was you pulled this up to plug the drain, then used both hands to turn on the water ( to get the temp. you want ). You then wash your hands in the standing water, then pushed the stopper down and let the water down.
To me it looks as if someone had their hair done and just set the paper down. Just because you are in a mental hospital dosn't mean that you have to look like crap. It helped the moral of the patients, who dosn't want to look pretty
